Start date or shifts: January 6 ASAP (3 months with option to extend)
Daily schedule: Monday to Friday, 8:00 AM 5:00 PM
Practice setting: PACE Clinic
Types of procedures: Comprehensive geriatric assessments; management of chronic conditions such as diabetes, hypertension, COPD, and CHF; medication reconciliation; coordination of care with interdisciplinary team members; annual wellness visits; acute care follow-ups; advanced care planning
Number of patients: 2 5 per day
Ages: 55 and older
Additional information: Provider will work within a multidisciplinary team that includes pharmacists, occupational and physical therapists, nurses, and social workers. The clinic operates alongside an adult day care center and offers home health for patients unable to travel. The site uses Epic EMR. Lunch is flexible around 11 or 12. Credentialing typically 30 90 days. Full travel covered (local candidates preferred).
