Specialty accepted: Family Nurse Practitioner, Adult Nurse Practitioner, Geriatric Nurse Practitioner, Physician Assistant
Start date or shifts: ASAP ongoing
Daily schedule: Monday through Sunday 7:00 AM to 7:00 PM. Five to eight assessments per day. Assessments last approximately forty five to seventy five minutes with last appointment scheduled at 6:00 PM.
Practice setting: In home health assessments for health insurance plans
Types of procedures: Health and wellness assessments, history and physical evaluations, medication review and reconciliation, physical exam, blood pressure check, finger stick, urine sample collection, fecal kit distribution for patient mail in, wellness and prevention education
Number of patients: Five to eight assessments per day
Ages: Geriatric population
Requirements: Must have CAQH, NPI, unrestricted state license, DEA not required, NP or PA nationally certified, minimum seventy five mile travel radius required, must provide monthly availability four weeks in advance, must commit to three consecutive days per week every week or two full weeks per month, strong computer skills and ability to work independently, white lab coat, stethoscope, ophthalmoscope, monofilament, tuning fork, reflex hammer, blood pressure cuff, pulse oximeter, portable scale, and tape measure required. Online OSHA and HIPAA certifications required before start. Must complete client training modules prior to scheduling. Current auto insurance coverage required. BLS certification required. Providers must not reside in the assignment state. Must be licensed in the highlighted state and at least one additional state (excluding CA, CO, CT, MA, ME, MN, NH, NY, RI, VT, WA). Neighboring state candidates preferred.
Additional information: Support provided through provider hotline. Client provides iPad, test strips, fecal kit, and PPE. Providers work independently as 1099 direct with client. Credentialing process thirty days. Mileage reimbursement and travel benefits available based on distance and length of assignment. Flights available if provider works a minimum of two full consecutive weeks. Rental car and hotel provided for qualifying assignments. Incentive pay and additional lab pay available based on performance. Providers must maintain cancellation rate below thirty five percent.

Benefits: Malpractice insurance coverage; weekly electronic pay; potential travel and lodging benefits; earn $2,000 per referral.
