Specialty: RN MDS Coordinator
Job Details:
Responsible for coordinating medical care, and matching patient needs and preference with judicious use of healthcare resources.
Reviews each resident s condition
Works in collaborative practice with Physicians and Physician extenders to actively support the delivery of cost effective, outcome oriented, quality care in the appropriate health care setting to meet patient specific, age and cultural needs.
Responsible for integrating, facilitating and coordinating patient care for high-risk patients across the healthcare continuum.
Manages patient care to prevent fragmentation and duplication of services.
Responsible for submitting necessary information to the Government.
Requirements:
2-3 years Previous MDS experience in an Inpatient/Acute Care Hospital Setting - Required
Experience as a Supervisor
Must possess a current, unencumbered, active license to practice as a registered nurse in this state.
Experience with all patient populations Required
Resident Assessment Coordinator (RAC) training must be completed within six months of hire. RAC Certification is preferred.
